A rather stout perennial with a leafy flowering stem 8-23 cm tall, growing from a firm, ovoid tuber and a second withered one.
Ecology UK: (distribution according to flora) It is a rather common plant of basic grasslands throughout England and Wales [but undergoing a dramatic decline], but becomes rare in the north.

Flowers: 12-15 mm diam., in a short, rather dense raceme, subtended by a linear, acute bract as long as ovary; outer perianth lobes purple, with conspicuous green veins, ovate; inner oblong, acute, all connivent to form the helmet; lip purple with a central line or dark patches, broader than long, about as long as the outer perianth lobes, three-lobed, lateral lobes crenulate, recurved, broad, cuneate, central lobe about the same size, notched and crenulate; spur stout, almost straight, about as long as the ovary, blunt, furrowed or notched at apex;
